If you have ever had a cold sore, you may be wondering whether lip filler could bring one on. The short answer is that lip filler does not cause cold sores in people who do not carry the virus. However, if you already carry the virus, any trauma to the lips, including injections, can trigger a cold sore Knowing why cold sores can be triggered after lip filler can help you prepare before your appointment and care for your lips after your procedure.
What causes cold sores?
Cold sores are caused by the herpes simplex virus type 1 (HSV-1). Once a person has the virus, it stays dormant in the nerve cells and can reactivate from time to time. The World Health Organization estimates that around two thirds of people under 50 carry HSV-1. Many don’t know they have it because they have never had a noticeable outbreak.
Common triggers of cold sores include stress, illness, fatigue, sun exposure, cold or windy weather, hormonal changes and physical trauma to the lip area (in this case think a needle entering the delicate lip area).
Why lip filler can trigger a cold sore
Lip filler cannot give you the herpes virus. What it can do is cause minor trauma to the lip tissue. Each entry point, along with the swelling that follows, puts the area under stress. In someone who already carries HSV-1, this can be enough to wake the dormant virus. A cold sore usually appears within a few days of treatment.
This is why your practitioner will ask about your cold sore history during your consultation and will always avoid performing a lip procedure with an active cold sore present.

How to prevent cold sores before lip filler
Preparation is the best way to reduce your risk.
- Tell your practitioner about any history of cold sores, even if your last outbreak was years ago.
- Ask whether antiviral medication before treatment is right for you. Your practitioner or GP can advise.
- Reschedule if you have an active cold sore or feel the tingling that often comes before one. Treatment should not go ahead until the area has fully healed.
- Look after your general health in the days before your appointment by getting enough sleep, managing stress and staying hydrated.
- Protect your lips from sun and wind with a hydrating lip product like the Arnica lip balm
- Start conditioning your lips early, because well hydrated lips are less likely to become dry and cracked.
Lip filler aftercare to support your lips
After treatment, your lips may feel swollen, tender and dry. Keeping the lip barrier hydrated and in good condition helps reduce cracking and irritation, which add further stress to the area, increasing the potential for cold sore formation.
The Arnicare Arnica Lip Balm was developed by nurses for post-procedure care. It combines arnica with hyaluronic acid to keep lips soft, nourished and comfortable while they settle. Apply it gently after your appointment and throughout the day.
Other aftercare for lip filler also applies here such as:
- Avoid touching, picking or rubbing your lips.
- Don’t share lip balms, cups or cutlery.
- Avoid saunas, extreme heat and intense exercise for as long as your practitioner advises.
- Stay hydrated and protect your lips from the sun.
- Contact your practitioner if you notice tingling, blistering or unusual pain.
Lip filler does not cause cold sores, but it can trigger one in people who already carry the virus. Good preparation, honest communication with your practitioner and consistent lip care afterwards give your lips the best chance of a smooth recovery.
